Primary Care Provider Services
Our team recognizes that there is a growing need for individuals to receive patient care in their own homes. Rather than taking the traditional route of going to a clinic, we offer a comfortable and convenient option without sacrificing quality of care.
Transitional Care
Post-operative, or transitional, care can help prevent complications or rehospitalization after a patient returns home. While transitional care can be short-term, our team will be there for you and your loved ones for as long as you need them.
Chronic Care Management
Our team understands that some individuals may need a more comprehensive care plan. Our providers work collaboratively with you to create individualized care plans. Our care team experts check in with you at least monthly to provide support and identify any issues that may arise. Chronic Care Management is aimed at decreasing complications, hospitalizations, and emergency room visits.
Medication Review with Pharmacist
Professional pharmacy services to review all prescription and non-prescription (over the counter) medications, vitamins and natural supplements. We want to ensure patients better understand how the medications interact with each other and other over the counter medications they may be taking. Assessing a patient's ability to administer medications and any potential drug therapy problems.
Annual Wellness Visits
We offer annual wellness visits for our patients, a yearly appointment to create or update your personalized prevention plan. This will also include any screenings or tests that you may meet the criteria for. We will also go through a "Health Risk Assessment" as part of this visit, a comprehensive review of your health that will allow us to develop the personalized prevention plan to help you stay healthy.
Preventative Immunization
All of our staff members are certified vaccinators. We offer a variety of preventative vaccinations including, but not limited to, seasonal influenza, pneumonia, and COVID-19.
